Consent to Join Event with Covid Concerns

BY REGISTERING FOR THIS EVENT YOU ARE AGREEING TO THE FOLLOWING:
If you do not agree to the following, DO NOT register.
By registering you agree that you and your guests understand the risks participating in a public event involve.

Our Measures: On Boats
Because the boat is entirely open-air, there is little risk.
The boat is washed down after every trip.

Our Measures: On Buses
Please be realistic, you will be on a bus.
This means you will be in close proximity to others and in an inclosed space.
We do not own all of the buses we work with and outsource to several companies.
We and the companies that we work with have established well-defined cleaning precautions.

By registering you are agreeing to and informing all participants you are registering for of the following:
- You agree to inform your guests of the risks listed in this consent.
- There are risks of exposure to and infection COVID-19 by attending ANY public event this one included.
- All guests understand Texas requires masks be warn in public areas and it is each guest's responsibility to do so
- Texas Winos & the companies it contracts with have taken preventative measures to aid in sanitation
- If you or any of your guests have any risk factors or symptoms, you agree to contact us to not attend the event

By participating you are taking a risk. A risk you do not have to take but are agreeing to. We have taken precautions but by participating you are agreeing that you are increasing your exposure to more people and therefore the chance of exposure to COVID-19 and will not hold this company or any partner company responsible for exposure to or infection of COVID-19.

THE LONG VERSION:
I understand that the Novel Coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization and the United States Centers for Disease Control and Prevention. I further understand that COVID-19 is extremely contagious and is believed to spread through person to person contact, and as a result, federal and state agencies recommend social distancing.

I understand even if I have been tested for COVID-19 and received a negative test result, in some cases, the test may fail to detect the virus or I may have contracted COVID-19 after the test.

I understand that COVID-19 may cause additional risks, some of which may not currently be known at this time. I understand possible exposure to COVID-19 before/during/after my transportation service may result in the following: positive COVID-19 diagnosis, extended quarantine/self-isolation, additional tests, hospitalization that may require medical therapy, intensive care treatment, possible need for intubation and/or ventilator support, other potential complications and risk of death.

I recognize that Texas Winos and its independent contractors are closely monitoring the situation and have put in place reasonable preventative measures including sanitation, disinfecting, distancing (where possible) and temperature testing practices. However, given the nature of the virus, I understand that there is an inherent risk of becoming infected with COVID-19. I understand all potential risks, including but not limited to the potential short term and long-term complications related to COVID-19, and have elected to proceed with my transportation service.

I understand the COVID-19 virus has a long incubation period during which carriers of the virus may not show symptoms (asymptomatic) and still be highly contagious. It is impossible to determine who has the virus and who does not given the current limitations of testing.

I understand that Texas law requires a mask to be worn. I understand this is for my safety and the safety of others with whom I am traveling and take responsibility for wearing a mask.

Each guest voluntarily assumes risk by attending any public event increase the chances of being exposed to or infected by COVID-19 and that such exposure or infection may result in personal injury, illness, permanent disability, and death. This is a recreational event. Each guest has understood the risks and has chosen to participate.

By participating in an event you are agreeing to release Texas Winos their employees and independent contractors from any legal liability for health issues, complications or injuries related directly or indirectly to the COVID-19 virus.

I also agree that if registering for others, I will inform them of the risks and precautions taken but that ultimately responsibility relies on each individual. You are taking responsibility for your guests to be informed of this.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my guests or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my guests may experience or incur in connection with my guest's attendance at the event.
